You fill this out and then you have to be patient.

Instagram account suspended - How long does it take to reactivate?
Unfortunately, there is no official information on how long it takes to get a response. Instagram may get back to you after a few hours or a few days. In the worst case, you may not get an answer at all. If you or a user who has your password have deleted your account, you can not reactivate it. It is not possible to restore the account. By the way, if you want to create a new account, it is not allowed to use the email address of a deleted account again.
